HOME HEALTH SERVICES
What is Home Health Care?
Simply put, Home Health is a health care services delivered to you at your own home. Following an illness or injury, you may need intermittent care at home as you recover. Home Health follows a physician's home health order, and is a level of service provided by licensed medical professional that includes:
- Home Health Aides (HHA) who can assist with activities of daily living such as bathing, personal hygiene and dressing, light cleaning, and exercises as outlined in your plan of care. The Home Health Aide will be under the supervision of the RN or PT.
- Skilled Nursing - Registered Nurses (RN's) and Licensed Vocational Nurses (LVN's), the nurses works with your doctor to provide you intermittent skilled nursing services such as wound care,post surgical care, chronic illness management, education about your medications and diseases so you can remain at home as you recover from an illness, maintain your independence, manage your disease and have fewer hospitalizations .
- Physical Therapists (PT), Occupational Therapists (OT), will come to your home as ordered by your doctor to provide therapy such as exercise, gait training, balance and fall prevention,and instruction on the use of durable medical equipment. A Speech Therapist (ST) can come who will work on your swallowing, voice or speech disorders as ordered by your doctor.
- Medical Social Work- A Medical Social Worker (MSW) can assist in Advance Directives, community resource referrals, long range planning, caregiver stress, short term counselling.
Using a team approach, we will work with you and your doctor to develop a care plan that will help you reach your optimal level of independence.
